Childhood Immunisation Outreach service
The aim of the national Immunisation programme is to reduce the impact of vaccine preventable disease by increasing the coverage to 95%.
The service is for children and families within North East Lincolnshire who have not accessed routine childhood immunisation because:
- They have not been located within a primary health care team
- The primary care team have not been successful in bringing the child/family into the primary care centre for immunisation.
Any child resident with North East Lincolnshire may be referred to the service by a health or social care practitioner- however stringent attempts should have been made to engage with the family and these will need to be detailed in the referral.
