We are a team of Family Support Workers who work to support local families from pregnancy until their children reach the age of five.
Families can be referred to the Family Support Team, by a referring agency such as Health Visitors, GP's , Midwives or families can refer themselves. We will then arrange a date and time to visit the family at home to tell them more about the service and the type of support we can provide.
What do we do ?
We work with families to prevent any difficulties that they may be experiencing from escalating. We can help families to identify their strengths, improve their confidence and gain control of difficult situations.
How do we do this ?
We work with families on an individual basis ti identify what the families feel that they need support with. The family will be assigned their individual family support worker who will visit them in their homes and work with them to design a plan of support. The plan of support will be reviewed by their assigned Family Support Worker on a six weekly basis to ensure that the families needs are being met, or to identify any change of circumstances which may require additional support. By working with the families to identify their own needs we support them in empowering themselves and finding their own solutions to the problems they have identified.
How can families access ?
For families living in Central East Manchester - families can access by going along to their local Sure Start Children's Centre at: Ardwick, Gorton North, Gorton South, Longsight , Levenshulme and referring themselves.
For those families living in Central West Manchester - families can access by going along to their local Sure Start Centre at: Moss Side, Hulme, Fallowfield, Rusholme, Whally Range and Chorlton.
